Greetings from Bangladesh. I m writing this mail to you to seek your kind opinion regarding the above infant who is my daughter. If I may be allowed to describe details from my perspective as follows which I believe may be useful . I realized out of discussion with local cardiac surgeon that it is a major surgical tasks and we would like to seek your views A. Second female child of the mother. CS baby with birth wt 2.26 kg. During 3rd TM, ultrasono report revealed no fetal abnormalities particularly heart and no congenital abnormalities. RVOT & LVOT normal. Heart rate was normal B. After 1 wk of age, she was OK. C. As on 15.2.12, she has had breathing problem with difficulties in coughing and unfortunately no attention could be applied towards heart until 3rd March when her heart rate was recorded 160/min. Color Doppler was done and found multiple ailments within the heart. D. She was then taken to Local Consultant on 6th March. After examination he asked for repeat Doppler (scanned copy is attached). Brief impression is as follows • DTGA • Large inlet, mascular and subpulmonary VSD shunting bi-rectional • PFO shunting Lt 2 rt • Criss-cross AV connection, straddling & overriding of mitral valve • Mod size PDA shunting Lt 2 Rt • Good bi-venticular Major problem as observed by mother: • Baby seems to vomit while on or after breastfeeding • Her appearance seems blue or black at wake up from sleep • Difficulties while crying or coughing My own observation: She is becoming black from a cuteness white child. Her b wt now 3.2 kg I’ll appreciate much a quick response. Best regards,
