Down With Flu, CBC Test Showed AML Type M1 And M2, Bone Marrow Biopsy Done. Advised BMT. Opinion?
my nieces is 24 years old, she was down with flu & fever and after few days back with CBC test, she diagnostic with AML Type M1 and M2,her bone marrow biopsy report details are below : 1-Lymphoid population: comprise 18% of total cellular events and predominantly composed of mature T- cells 2- Myeloid Population: marked decreased and comprise 05% of total cellular events. 3-Monocyte Population: Comprise 1% of total cellular events 4-Blast Population: increased at ~56% characterized as CD45++,CD13+,CD33+,CD34+(minor subpopulation) CD117+++, MPO+++ and negative for all other antigens . After that she had one round of Chemotherapy and the Flow Cytometric Immunophenotyping Test result was shown that “NO Residual Disease Seen” And for Bone Marrow Aspiration Test Shows following nucleated cell differential counts : Myelocytes 43% Metamyelocytes 02% Neutrophils 29% Plasma Cells -1% Eythroid Precursors 15% Lymphocytes 06% Dusplastic Cells01% and Blast 03% Iron Stain: there is increased (3+) stainable iron seen Now she had her consolidation phase with high dose of chemotherapy need to know does she should have BMT after that or no need of BMT because her Oncologist is suggesting BMT, she is young and she has to get married and whole life is ahead of her .therefore am looking forward for honest and expert second opinion for her further treatment. AML is an aggressive leukemia. Chemotherapy is used for remission of the disease.
However, due to aggressiveness of the leukemia, chemotherapy may not be able to achieve long lasting remission.
Regular flowcytometry should be done to look for minimal residual disease.
Thus, bone marrow transplantation (BMT) is the treatment of choice.
It is the treatment of choice to remove the clone of abnormal cells.
Before going for BMT, an appropriate donor needs to be arranged.
Bone marrow can be donated by relatives (especially siblings) or by unrelated people.
The pre-requisite for BMT is HLA matching.
When HLA matching is good, the life of the graft is good.
Quality of life further is also good when proper matching is done pre-operatively.
So, do not worry.
BMT is now being done successfully all over the world with good results.
