Fever, Tender Ankle Bone. No Swelling. Bone Infection?

Hello dr Goyal
My niece has been complaining of left ankle pain, she may have sprained her ankle. There is no visible swelling but some tenderness. But at the same time for the last 3 days she is running fever. She had low grade fever on Tuesday and we'd ( 99-101) but today her fever touched 103. We took her to the ER. The intern is speculating possibility of bone infection .

Hello.
Thanks for writing to us.
The tenderness in the ankle may be related to an ankle sprain but if she is having a considerable pain on weight bearing then a bone injury or infection needs to be ruled out after a direct examination and investigations

Radiologist Dr. Parag Goyal's  Response
There is a high chance of bone infection which in medical terns we call as osteomyelitis. Remote possibility could be some ligament tear. U should initially go for X ray ankle. If this is inconclusive, then go for MRI to reach the exact diagnosis.

Orthopaedic Surgeon, Joint Replacement Dr. Saurabh Gupta's  Response
Hi,

After going through the entire description, I am of the opinion that the cause for her pain is sprained ankle.

Next before you go to the doctor you should try the following :-
Avoid putting weight on your ankle as best you can.
Ice your ankle to reduce pain and swelling. Do it for 20 to 30 minutes every three to four hours for two to three days, or until the pain is gone.
Compress your ankle. Use an elastic bandage to keep down swelling.
Elevate your ankle on a pillow when you’re sitting or lying down.
Use braces or ankle stirrups to give your ankle support.
Take anti-inflammatory painkillers. Nonsteroidal anti-inflammatory drugs (NSAIDs), like ibuprofen, naproxen, or aspirin will help with pain and swelling.

I do hope that you have found something helpful and informative.
