How Serious Is The Placenta Previa?

Hi, I am 23 weeks pregnant and have been diagnosed with total placenta previa. I have been experiencing mild to severe lower abdominal pressure and I wanted to know if I should worry? Also I experience mild incontinence.Every time I cough or sneeze I secrete a small amount of urine.Does this have anything to do with my placenta previa?

Dear Madam,

Total placenta previa, what we call as Central placenta paevia is condition which needs serious attention. In this condition, the placenta is sitting on the mouth of the uterus in front of the baby. Thus, there is no possibility of baby delivering normally and has to be delivered by caesarean section.

Moreover, as the pregnancy advances, the mouth of the uterus (cervix) is getting merged with the uterus and thus the placenta gets separated. This leads to bleeding. The first bout of the bleeding may be minimal, however, the subsequent bouts are threateningly alarming. Since, the uterus is not contracting, the bleeding is painless and can not be controlled naturally. Being painless bleeding, many a times the husband sleeping by the side realizes the bleeding due to wet bed. Placenta paevia bleeding is described as painless, causeless and recurrent bleeding. Hence you do not know when the bleeding will occur. I would advice you to get admitted in the hospital till your delivery so that close watch can be kept.

When you go to toilet, please inform the nurse about it, do not latch the door from inside. Where ever you go from your bed, you must inform the nurse.

These patients invariably require blood transfusion. Hence, please keep few blood donors of your blood group ready.

May be the placental mass is giving pressure on the bladder causing the stress incontinence.

I do not want to threaten you, however, madam, you should be given complete, thorough and realistic information about the condition you are having so that you will take proper care.
