General Surgeon 's  Response
Respected Sir, hi I evaluated your query thoroughly.
Not mentioned present status of smoking but I guess it`s still going on.
This is the combined effects of vascular compromise through diabetes and smoking leading together accumulated symptoms and worsening.
Your surgeon is true regarding amputation level which will be higher this time according to the site of vascular blockage.
Thanks for using Health care Magic.
Regards, Bye
Take care.
