55 YR OLD MALE SPIDER BITE ON SAT. BITE ULCERATED ON SUN GAVE ONE DOSE BACTRIM BY 3PM HE HAD RAISED NODULES/PUSTULES ON ENTIRE LEFT SIDE OF NECK AND HEAD AND SWOLLEN LYMPH NODE WBC OF 8.8. IN ER I ASKED THEM TO LANCE AND CULTURE, THEY SWITCHED HIM TO CLINDAMYCIN. THE INFECTION WAS STAPH AUREUS SENSITIVE TO ALL ANTIBIOTICS. YESTERDAY WBC WAS 3 SLIGHT SHIFT NO BANDS. HE NOW HAS SLIGHT FEVER 99.9, THERE ARE A FEW PUSTULES ON HIS CHEST AND FOREHEAD AND ENTIRE LEFT NECK REGION IS COVERED. LF=EFT NODE IN ROCK HARD AND EAR IS SWOLLEN AND PUSS. HE HAD LEAKY VALVE AND CONCERNED OF COMPLICATIONS. ONE DOC SAYS NO PROBLEM ONE SAYS FURTHER EVALUATIONS NEED TO BE DONE. YOUR FEEDBACK
It seems that this infection is moving along very fast and I too am concerned about further complications. I do believe further evaluation is needed. If he has a leaky valve I would be very concerned about the possibility of the valve being infected with the staph aureus. The spider bite per se doesn't need further treatment. It seems like it might have been the source of the original infection, but now the staph is within his body and spreading quickly. Based on what you are saying here, it sounds like he needs aggressive antibiotic therapy and may need to be hospitalized. Certainly I would be checking his heart with an echocardiogram to see if the valve is involved. Hope this helps.
