Dear sir i understand your concern.I am sorry to hear you have been having difficulties getting your blood sugar in the normal range now. I am very happy you are this conscientious because having a well controlled blood sugar is very important in preventing the onset of diabetic complications.
ok now i will explain you how to adjust your insulin in fact its very easy sir.
60% of your daily insulin requirement will be from lantus since its basal insulin and work for 24 hrs and rest of insulin will be just before food three times.we need to keep you blood sugar below 10 after food and below 8 during before break fast.if fasting sugar is not under control you can increase lantus by 2 units and see for two days.if fasting below 8 its ok other wise you need to increase lantus by 2 units and watch for two days with same dose.
2. To adjust humalog dose, your after food blood sugar is guide.if after 2 hrs after taking food sugar is not below 10 try increasing the same insulin by 2 units for next day.for example to day you took 23 units before breakfast and your sugars after breakfast are 12 and before break fast are 8 then.tomorrow you will increase your humalog to 25 units ...ok
I suppose I have given the desired response if you have any other query please write back.
Otherwise please accept & rate my answer. I will appreciate your review about my answer.
Best Regards.
