Hello,
Welcome to healthcare magic.
From your description, you seem to be having grade 3-4 acne.
You may benefit from taking oral isotretinoin. This is a prescription drug which should be taken strictly under the guidance of a dermatologist. You could also benefit from oral doxycycline or azithromycin.
You could apply gel containing clindamycin in the morning and 2.5% benzoyl peroxide at bedtime. You can use 2 % salicylic acid containing face wash.
Drink plenty of water and have a healthy diet.
Take care.
