The drug in question here is an Oral Retinoid i.e
Isotretinoin.
It is a
Vitamin A derivative and is one of the most effective and safe antiacne medicine that has yet been known.
Isotretinoin is US FDA approved for
severe acne.
Acne associated with significant physical or mental scaring, acne not responding adequately to traditional antiacne treatments like
oral antibiotics and topical's are other indications in which Isotretinoin is indicated.
Dermatologists have been using Isotretinoin over the years with success and consistently reproducible results.
Common side effects are minor and limited to the skin i.e dryness of lips and skin in general, increased hair shedding in some but not all; these are reversible once treatment is over.
The most significant of the side effects is fetal malformations in pregnant females, therefore, pregnancy is strictly contraindicated while on treatment with Isotretinoin and for 1 month after discontinuation.
Some elevations in serum lipids like triglycerides and liver enzymes is common but rarely of any consequence, however they ought to be monitored every 2 weeks initially and thereafter every 2-3 months. Those who have raised baseline levels require more stringent monitoring.
Serious side effects like
hepatitis.
pancreatitis are rare.
Overall, it is one of the most effective treatment for difficult to control acne and it is first choice in
nodulocystic acne, acne associated with significant psychological distress.
