Is Abortion Recommended While Having Placenta Previa?

My sister is pregnant with her third child, with this one she has been diagnosed with placenta previa, and the doctors are warning her about the risk that she will have with this one.. she is a paramedic and a single mother of two they advised her to be on bedrest, but for the survial of her lil family she cannot be on bed rest... is it possible to have an abortion if you have the placenta previa... what are the risks,.... and when is it to late. thank you!

Placenta paevia is a condition wherein the placenta is located in the lower part of the uterus and it is ahead of the head of the baby. Since the lower part of the uterus is continuously retracting, there is always a risk of the placenta getting separated which will lead to a life threatening bleeding. Also it may obstruct the head of the baby during delivery and will keep on bleeding requiring Caesarean delivery to save mother's life.

Usually, the first episode of the bleeding is comparatively smaller one; however, the subsequent episodes may be very heavy.

Bleeding of placenta praevia is typically described as causeless, painless and recurrent bleeding. In short, there is no clue to predict the episode of bleeding. Hence, the mother has to be under observation 24x7 lest the risk of collapsing without anybody's notice. So your sister cannot continue to work with placenta praevia. If she starts bleeding heavily at her workplace, she cannot get the medical help which may include immediate surgical intervention and many units of blood transfusion in time.

I do appreciate your concern and empathize her situation. Your option for terminating the pregnancy also is understandable. However the million dollar question is: what is the duration of her pregnancy?

In most of the countries, termination of pregnancy is illegal beyond 20-24 weeks of pregnancy. If she is within the legal limits of the termination of pregnancy in the country where she is residing, she may opt of termination of pregnancy. However, method for termination will mostly be surgical one (depending upon the grade of placenta praevia).

I feel, you need to discuss this issue with your obstetrician and finally take the decision. The decision will depend upon the following issues:

1. What is the grade of placenta praevia?

2. What is the period of gestation?

3. What are the facilities available in the place where she is hospitalized?

4. What is the risk to your sister's life in continuing the pregnancy?

5. What is the opinion and skill of the Obstetrician?

6. In which country is she residing?

So please discuss the whole situation in details with your Obstetrician to take a safe decision.
