The
ultrasound report suggests:
The scrotal sacs are empty.
The two testis are not in their normal position.
The two testis are placed in the bilateral inguinal canals and there is associated inguinal
hernia on left side.
It is quite strange that this condition is detected so late i.e. age of 45 years.
Absence of testis in scrotal sacs and presence at some other site is called
cryptorchidism.
The testis when present in abdominal cavity have to be removed early in life because high temperature of the abdominal cavity can cause permanent damage to the testis.
The abdominal testis if not replaced early in life are removed surgically, because these testis have high chance of
malignant transformation.
In your case the testis are in the inguinal canal.
The testis in inguinal canal also have high chance of malignant transformation.
Thus, their removal is advisable, since, the testis would have become sterile by now and have no role in secretion of sex hormones.
Replacing the testis in place will have no benefit since the testis are sterile and replacing them back will not reduce the risk of development of malignancy.
