Fluid in the Douglas simply means that there is a fluid in the abdominal cavity. There might be many reasons of it. And it is impossible to tell according to your ultrasound.
Fluid in the abdomen appears due to inflammation, low protein level or due to a cancer. It can be an appendicitis, ovarian cyst, ovarian inflammation, intestines inflammation, pregnancy, ovarian cancer, pancreas cancer, stomach cancer, large intestine cancer, liver cirrhosis and many many more.
I suggest you to have some blood tests. Blood work, CRP, CA 19-9, CA 125, CEA, liver tests, proteins level, pregnancy test, urine test should be done at first. Next step is abdominal CT.
