If I were your treating doctor for this case of severe pain & inflammation of joints , I would come come up with these possibilities, these includes.

Rheumatoid arthritis , diagnosis can be confirmed by C B C , E S R ( 40 mm ) , rheumatic factor.
Consult a rheumatologist and get his opinion and treatment .As there are
severe pain in numerous joints , swelling with
redness ( inflammation ) raised E S R and involvement of many joints , is an indication of rheumatoid arthritis.

Osteoarthritis your age is against ( but can't be ruled out) . usually osteoarthritis involves larger joint first but exception may be there . Diagnosis can be confirmed with consultation with an orthopedic surgeon.
3 Gouty arthritis , diagnosis confirmed by blood examination for uric acid level, if found elevated then chance of gouty arthritis . Consult an orthopedician and get his opinion.
4 In acute cases chickenguniea may be responsible for joint pain , so if
fever history present then consult a physician.
So consult a rheumatologist and get his opinion first of all.
