Pulsating Headache. MRI Reveals Pansinusitis With A Small Sphenoidal Polyp. Diabetic And Hypertensive

I am 42 yr old male.diabetic and hypertensive since 10 yrs.. iam having unilateral, right sided, band like sometimes pulsating, moderaye to severe headache since 15 days.it is relieved by taking analgesics or sumatriptan promptly for 6to8 hrs, but agin recurs as the effect of drug wanes off.
I am on telmisartan40...metformin 500...od with a good control of my comorbidities.
20yrs back I had a same type of headache for one month...I tried multiple drugs but was relieved with 3 doses of dexamethasone for 3 days
MRI...minimal pansinusitis with a small sphenoidal polyp...but I dont have any symptos of sinusitis

Hi,
Thank you for posting your query.

Most likely, you have a flare up of migraine, and you may respond to dexamethasone once again.

However, in addition, I would also recommend you to be on preventive treatment for migraines, to reduce the severity and frequency of headaches. Options include beta blockers, topiramate, divalproex, etc.

Pansinusitis on MRI may not mean that you have symptoms of sinusitis, and that need not be treated.
