Suggest Dosage Of Insulin To Control Diabetes

Hello, I have guest in my home from Serbia, he is 6ft 2 , and weighs about 220 lbs. He takes Novolet 30 (pen) insulin 2 times daily, in the morning he takes 28 and in the evening 14 - and it s not working out very well - we are having many problems and hospital visits. Is there a correct chart as to how much insulin he should be taking that we can follow and control his insulin much better? Thank you for your help.

Hi.

I can understand your concern.

Your guest is 6ft 2 and weighs about 220 lbs, So his BMI is 28 which makes him Overweight according to guidelines. So wight reduction would be the first advice for him. Weight reduction by Diet Control & Exercise will also help him in control of his blood sugar levels.

Regarding Dosage of Insulin -
As you have not mentioned values of his blood sugar levels, it would be highly inappropriate to advice his insulin requirement without knowing his Sugar levels.

But I can assure you that even with his current schedule of taking two injections (Morning and Evening Inj), his sugar levels can be controlled by modifications in Insulin Units depending on how high is his blood Sugar levels.

If I would be his treating diabetologist I would advice him to Check for his early morning Fasting Blood sugar levels (FBS), Post Prandial Blood Sugar (PPBS) & HbA1C (3 month Average Blood sugar), Based on these investigations his insulin requirement should be adjusted.

Hello,
I'm afraid there isn't anything like that. Diabetes control is a complex task that may take some time. Most of all, it requires patient cooperation.
Is his diabetes type 1 or type 2?
I understand he is overweight which probably means that he likes eating. Insulin dosage may vary a lot depending on food intake (quantity and quality), physical activity and intrinsic factors like the patient's resistance to insulin.
He should cooperate closely with his doctor to create an eating schedule, engage in physical activities if not contraindicated by other conditions and learn to modify the dosage of insulin depending on food, activity and blood glucose measurements. This is too broad to be covered here in detail.
If you need more information then please provide more data like the patient's history (including his age and type of diabetes), drugs, eating schedule, measurements (fasting glucose, before meals and 2 hours after them) and his activities schedule as well.
