i am Dr.zuata recently having acute backache the MRI shows extruded disc at L5 and prolapse at L4 level.Although very acute at onset i am improving very much now with no either motor or sensory loss at all.Still My Neurosurgeon advise me that a undergo Micro Lumbar discectomy.So what would be the best line of management as of now since surgery is almost always the last option after all the conservative Rx fails as tha book says.and what is the chance of its recurrence with or without surgery.
Hi Welcome to healthcaremagic After going through your query I came to know that you are probably suffering from lumbar disc prolapse. It is confirmed by MRI scan. There are two treatment options, rest and surgery.You are correct in thinking that surgery is last option. I think you should continue with conservative treatment.Conservative method is rest in position of relief(generally lateral position with both knee bend),neurotopics such as mecobalamin and analgesics (Diclofenac three times a day after meals is effective). Sometimes strong analgesic(such as tramadol three times a day after meals) is required. Omeperazole before meals prevent acidity caused by analgesics . This method needs patience as it take longer treatment times sometimes up to 6 months. You asked about recurrence , so recurrences are mostly in two years whenever one exert more at spine surgery or not. Within 2 years disc generally settle and recurrences are less. According to books results of both surgery and rest are same after two years . You can discuss with your treating Doctor about it. Hope your query get answered.
