Suggest Treatment For Sharp Pain Caused By Osteoarthritis

Thank you for your time. I have osteoarthritis and have had bumps and minor pains for years. In the past month my left ring finger draws up horribly during sleep and now 24/7. I can straighten it out somewhat but only with sharp pain and the slightest movement at times is just excruciating. Thirty years ago I had carpal tunnel surgery and my thumb was damaged and surgery on that tendon (?) followed. The thumb would jump out of socket and that is how this finger thing has started. It does seem that the right hand is stiffening more now as well. After all that - do I see a rheumatologist or orthopedic/hand specialist? It seems impossible to get a live person at local offices.
