Swelling And Pain In The Foot. Biopsy Done. Hole In The Foot Diagnosed. Cause?

My niece had been experiencing foot swelling and pain in the upper part of her foot. She had an incision biopsy done. Orthopedic surgeon said there was a hole in her foot one at the top of her foot before the toe bones. What could cause a hole in her foot bone? She has never had an injury to this foot. She is 41 years old per menopause.
