Swelling In The Chin After A Tooth Extraction. Is This The Hematoma Formation?

i have a hematoma from a tooth extraction that occurred on tuesday. the dentist took me to the ER and they called an oral surgeon who said there was nothing they can do about it, except what I ve already read, heat, compression, etc. The hematoma is not getting smaller like they said it probably would. This morning I woke up and it has come down into my lymph node. I can feel that the swelling is now under my chin when yesterday it was just on my cheek. Should I go back to the ER? I hate to do that if there s nothing they can do. What do I do???

hi and welcome ,
After extraction it takes 5 to 7 days for complete healing of the extraction socket under normal circumstances if there is no super infection of the wound or dry socket .
Hematoma can be seen post extraction only when the extraction is traumatic leading to damage to underlying blood vessels or other vital structures.
This can be evaluated only on clinical examination of the lump and associated x rays . i suggest you to get can OPG radiograph done which will show the exact relation of the swelling in relation to teeth , chin and other areas. and approach an oral surgeon who will come up with exact treatment plan .
Mean time you can continue with antibiotic and analgesic course to prevent the super infection of the wound. take care.

Thank you for the query.
Usually traumatic extractions lead to complications such as hematoma formation which is due to collection of blood outside the bloodvessel.
Palpable lymphnodes are due the infection .
Treatment of hematoma is done by applying cold packs to produce vasoconstriction which helps to decrease the hemorrhage and swelling.
I would advise you to see your dentist to rule out the exact cause for the infection as your lymph nodes are palpable .
A course of antibiotics and analgesics will help in reduction of pain and infection.

Hi,
Welcome to the forum.
with the details provided by you, I suspect that the decay has reached the pulp of the tooth (part of tooth where nerves and blood vessels are present). You need to get an X-ray done and take an opinion from an specialist. Even if it is a nerve problem, nerves will not be visible through the chin or cheeks. probably you have seen the veins which generally becomes visible when there is swelling. Do take second opinion from an specialist (endodontist )

...this can be due to the faulty restoration given to the tooth .first take an x-ray of the filled tooth.check for any periapical radiolucency or dark in the area of root.if the filling has given without removing the decay fully or without treating the root infected,it can lead into swelling around the tooth and irritation of nerves around it.please visit your dentist at the earliest and ask whether root canal treatment is needed for the tooth .hope you will get well soon.thank you

If you are having swelling creep down your neck, I strongly suggest going back to the ER. It sounds like you have contracted an infection and it is spreading into the spaces of your neck. This can be life threatening if not addressed early on.

Swelling is other common complication after tooth extraction. Swelling after tooth extraction procedure is a normal body reaction and is indicator of normal progress of healing of tooth extraction socket. If swelling persists beyond 1week, then the patient needs to visit the dentist. After extensive surgical interference, simple inflammatory edema appears in adjacent tissues in most of the cases. This is unavoidable and is generally commensurate with the degree of surgical trauma. Swelling usually occurs after the tooth removal. The swelling can be there for as long as few weeks. The swelling can be treated by using an ice-pack or by pressing a bag of frozen peas against the cheek near the tooth which is removed. This will help to lessen the swelling. Alcohol, hot foods and drinks and work out immediately after the tooth extraction can aggravate the swelling. Warm saline rinses and hot moist compresses 1-2 days after tooth extraction will further help in reducing the swelling.
