Throbbing Pain In Tooth And Gum, Filling In Tooth Started To Rear Apart. Is My Tooth Infected?

I had a root canal done about 3 and a half years ago when I was under my mothers dental insurance, and got a temporary filling and never came up with the money to get it caped. Now my temp filling has slowly started to rear apart in two spots and in the past week I started having a throbbing pain in my tooth and gums. Is my tooth re infected? Do I need to go to the emergency room or something? I have no health insurance and recently lost my job so I have no sorce of income. I am unsure of what to do. If its a minor thing I can put it off until I get a job but if it is major my only option is the emergency room.

Hello and welcome to HCM forum,
I would like to inform you that throbbing pain and swelling in a root canal treated tooth is from re-infection. Your tooth needs a dentist attention, as the root canal treatment will have to be performed again in order to remove infection and fill the tooth with fresh filling material.
I will advise you to see your dentist, but if seeing your dentist is not an option for a while then i will advise you not to chew anything from the affected side.
Take antibiotics and pain killer if possible , after every 8 hours.
do not overdose with medication due to side-effects.
Also, you can perform warm saline rinses 3-4 times/day,
Antibacterial mouth rinse will also help in relieving pain and swelling to some extent.
Kindly visit your dentist whenever its possible as these instruction will provide temporary relief only.

Hello,

Welcome to healthcaremagic.

Since the pain associated with root canal treated tooth, most probably the involved tooth may be re-infected.

Based on clinical as well as x-ray evaluation,tooth has to be either re-rc treated with crown placed.

If tooth has grossly infected,get it extracted.

Usually in such cases ,amoxycillin as well as analgesics are usually prescribed medication.

I would advice you to visit the dentist and get the required treatment done without fail.

welcome to HCM. Once a root canal treatment is correctly done a permanent filling like amalgam or composite resin filling should have been placed until the tooth is covered by a crown. It is surprising why your RCT treated tooth was covered by a temporary filling. Anyway once the filling is fully or partly lost, there is seepage of saliva through the lost space of filling and that slowly dissolve the cement used to seal the filling material(gutta percha) in the root canal.Naturally bacteria from oral cavity again enters into the canal and sets up a new infection focus. Which again causes pain and other symptoms of infection. If you do not have any other decayed tooth beside your RCT treated tooth or if you do not have infected gum, then your RCT treated tooth is the cause of your recent throbbing pain. ER will not be able to do much. They can prescribe you some antibiotics and analgesics to relieve pain. For temporary relief you can start taking antibiotics like Amoxicillin 500mg + clavulanic acid 125 mg( eg Augmentin) 8 hourly for 5 days and Paracetamol 1 gm as and when you feel pain. But you have to visit to your dentist for Re-treatment(a new root canal treatment of that tooth) and crown of that tooth to get permanent relief.

in your case there r two possibility
1. re infection
2. fracture of tooth

as per yr clinical history more chances of reinfection. so u have to visit dentist and at least do re RCT + permanent feeling on that. and when u get job do crown on that otherwise again chances of fracture of that tooth because after rct tooth become brittle.

Thanks for asking your query,

Yes your teeth is re-infected and in fact it is an emergency.Root canal treated teeth need a crown(cap).since you delayed for the crown ,root canal treated teeth got reinfected.your dentist is the right person to tell you what needs to be done now.he might take a few x-rays and he will put you in antibiotics and analgesics immediately since you have throbbing pain.

In the absence of root canal therapy the alternative is extraction. but if you don't get treated either way The infection can also spread to the bone around the tooth leading to formation of abscess.Untreated abscesses may get worse and lead to other complications.

Talk to your dentist regarding your financial situation,he might address your problem.he can suggest you few options that can be beneficial to both.
