What Causes An Abnormal Period With Heavy Bleeding?

I am experiencing an abnormal period, going through a super plus tampon every 20 to 30 minutes. I am 48 years old and am peri-menopausal however I dont have a regular period and have not had for several years. I may have a period this month and no more for sometimes 3 months.
