What Causes An Itchy Rash On The Penis?

my 2.5 year old son has a bad rash on the back side of his penis. He scratches it at nights and woke up this morning with the rash bloody. Previously his Dr said it's a fungal infection and prescribed simple OTC antifungal cream. This doesnt seem to help. He's got mild allergies and taking Singulair. is there potential medicine issue?
