What Causes Gnawing Feeling In Stomach?

Hello, ive been having a terrible gnawing feeling in my stomach for about 30 minutes. Before that, I havent ate nor drank anything all day till around mid afternoon. Before I ate (which was 30 mins ago) I got a terrible tightening and gnawing feeling in my upper abdomen around under my breasts. A few mins later I started getting a burning feeling that radiated all over my stomach. Ive been burping a lot but my stomach feels a little bloated and tender. I do have a history of acid reflux but this time, it felt way different. What could it be and should I be concerned?
