What Causes Lacerated Median Nerve After Carpel Tunnel Surgery?

A friend of mine had his median nerve lacerated during endoscopic carpal tunnel surgery. The doctor is now claiming that it was due to preoperative adhesions. The doctor says that the adhesion caused the nerve to stick to the ligament. If this is true, should the surgeon have seen the adhesion with the scope during surgery. The surgeon never mentioned "adhesions" until my friend filed a lawsuit
