Hi,I am Dr. Santosh Kondekar (Pediatrician). I will be looking into your question and guiding you through the process. Please write your question below.
Dear Dr. My son (who is currently 3 yrs 6 months old) had sudden spike in temperature & had febrile seizure 4 months ago. The episode lasted for 20mins before i cud reach him to the hospital. Doctors suspected Leptospirosis based on his blood results. Post his recovery (total 9days of hospitalization) and after analyzing his CT (which showed normal) and EEG (large variation - taken on 2nd day from Episode) he was advised to start EPTOIN (oral susp: phenytoin) for 6 months as per the Neurologist. But after this incident he gets fever once in 15 days. He was advised to take antibiotics for a week and the fever will subside. This pattern is continuing for last 4 months. Recently he got high grade fever 103 degree and lasted for a 7 days inspite of antibiotics. His blood counts were analysed - NO - Leptospyrosis, NO - malaria, NO - Typhoid - but showed some bacterial infection - so was in monocef iv for 2 days - fever came to normal on 9th day but developed skin rash which lasted for 2 days. Doctor said rash could be post viral fever effect and antibiotic was stopped. Even Abdomen USG was normal. Previously he was anemic but improved over the last 2 months and on a positive note. He is doing well for last 4 days. Paed' Neurologist has advised to stop EPTOIN and use frisium tablets in case of high fever and do towel bath to reduce the fever in future occurrence. He is going to kinder garden - active kid - never complaint of fever over last 3 years until this Seizure episode. 1. What could be the reason for his frequent fever and particularly happening once in every 15 days. 2. Is his immune is very low so that he catches fever from his school often 3. Are we missing any particular diagnosis for my son. 4. Or is it a normal issue with kids of his age going to school. Pls advice Thanks
Hai...first of all I would comply with the neurologist decision of stopping eptoin. Coming to tour questions... 1. It could be recurrent viral illness. 2. Immunity is perfectly fine as he hasn't gotten into serious bacterial infection till now. 3. I would like to know more about his growth and development and family history of seizures...before answering if we are missing anything. 4. You are right...kids usually have 2 periods of recurrent infections in early life....once when they are newly joining school & another during college freshman days.
