Hello,

Since the pain is so much unbearable that your friend is crying it is necessary to take him to the emergency room for clinical evaluation, examination, and investigations. It may be due to ureteric colic or intestinal obstruction or enterocolitis. If there is no nausea or vomiting give him an antispasmodic and see the results. If there are associated urinary symptoms like hematuria, frequency or burning, ask him to take plenty of oral fluids. Talk to the doctor anyway.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant
