You were prescribed effexor a couple months ago for depression. You also take keppra, depakote, and lamictal for seizures, recently started taking l citrulline and pycnogenol for nitric oxide for healthy endothelium system. You are 40 yrs old 6 3 185 lbs. blood pressure is usually around 118/80. All these indicate that this is basically an excessive sweating, that might have caused the dermatitis of the scrotal skin and hence the excessive discharge and wetting of the undergarment. I would advise you the following: Consult a Dermatologist for clinical evaluation, examination and proper management. Apply liquid povidone iodine to the whole area twice daily so that the area is at least sanitized to avoid the secondary infection, add on oral antibiotic, metronidazole in low dose and multivitamins to give strength to the skin.
