Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Local genital examination is necessary in your case. Phimosis and paraphimosis should be ruled out by examination. If any surgical cause found then get it treated accordingly with surgery.

If no specific underlying cause present then balanitis is the cause and for that topical antibiotic and anti-inflammatory oral drug can be prescribed. Consult a urologist for examination and discuss your case.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Parth Goswami
