welcome to Health care magic.
1.Osteophytes are bony growth in degenerative changes causing the nerve root compression.
2.Bilateral herniation of the disc posteriorly causing the nerve compression which is passing through the foramen.
3.Posterio centrally there is a ligamental mass effect of the disc.
4.No surgery is not always indicated. Initially the treatment is exercise and physiotherapy.
5.Only when it does not work one will plan for surgery / intervention.
6.In your case initially physiotherapy will be tried.
Hope it helps you. Wish you a good health.
