Hello and welcome to HCM,
The creatinine levels and total proteins in urine are raised.
Raised blood creatinine and urine proteins suggest deranged renal functions.
Since there is history of hypertension, the renal dysfunction could be due to the long term effect of hypertension.
The kidneys on imaging suggests reduction in the thickness of renal parenchyma.
The reduction in thickness of the renal cortex could be due to the effect of hypertension.
You need to consult your treating doctor as well as nephrologist for further management.
A change of current treatment may be required.

Thanks and take care
