What Does Prominence Of Ventricular System And Sulci Mean In An MRI?

hi I was diagnosed via MRI with MS last July 2013.. I am a 57 year old woman and have severe gait issues as well as having a recent MS flair that my doctor treated with IV steroids. I have just had a new MRI of my brain (along with several Evoked Potential tests) and in reading the report from the recent MRI -- I see the following terminology Prominence of the ventricular syystem and sulci compatible with volume loss. Redemonstrated are disctrete and confluent areas of T2 prologation in the periventrucular deep and subcortial white matter as well as the pons and left cerebellar hemisphere ...without significant interval change..the findings are non specific and may reflect sequelase if chronic microvascular ischemic change or demylenation. Flow voids are maintained within the anterior and posterior cuirculation..there is vertebrobasilar dolichoectasia.. Im trying to see if this all means that I might have moved to Secondary Progressive MS from the RRMS that I was originally diagnosed with

MRI findings are due to age related atrophic changes and chronic microvascular ischemic changes resulting from involvement of small blood vessels.

Age related atrophic chanes causes loss of brain parenchyma and it causes prominence of the ventricular system and sulci.

Regarding chronic microvascular ischemic changes, it is important to rule out demyelinating disease with follow up scans.
Ischemic changes resulting from blood vessels changes in diseases like hypertension,diabetes mellitus,dyslipidemia.
So,you need monitoring of conditions that leads to ischemic changes like hypertension,altered lipid profile,diabetes mellitus.
You need investigations like routine hemogram,RBS,LFT,RFT,Lipid profile,ultrasound of abdomen.
Proper treatment depend upon findings.
