Ill defined lesion in right frontal region and basal ganglia may be due to many reasons like infarct, gliosis, tumour etc. From history(left side paralysis),it appears that you may have findings due to decreased blood supply like infarct. So,you need monitoring of conditions that leads to ischemic changes like hypertension, altered lipid profile, diabetes mellitus so that further progression of disease can be halted. You need investigations like routine hemogram,RBS,LFT,RFT,Lipid profile, ultrasound of abdomen. Treatment depend upon findings.
Other findings are normal. You should continue physiotherapy.
