Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.
The report says all of your sinuses are inflamed along with with nasal turbinate grown inside along with small polyp in your maxillary sinus. Your nasal septum is deviated to right side with very less access to upwards into the nasal cavity. Basically you have severe nasal septum deviation with sinusitis which makes your headache and nose block. Further contact with your ENT is necessary to have appropriate medication or any surgery followed.
Hope I have answered your query.
