What Is The Reason For Consistent High Sugar Despite Taking Medicines?

I am diabetic type 2 , and I am taking Glimepiride twice a day (total 4 mg) , I used to take one a day (2 mg) , but I complained high sugar , and my Doctor doubled it. I am still taking high number when I measure my sugar in night ( that is the time I measure it) like 160, 180 200 and highest I ever had was 266 list night. I called my Doctor office , he recommended Metformin. I don t like this medicine , because it created side effects like headache, stomach upset and so on. I am confused now. I exercise , I watch my diet still my sugar is high . Please tell me what triggers this . I appreciate your advise.

Dear sir

The perisistently raised high blood sugars are due to mainly two reasons
1. Increased hepatic glucose output
2. Decreased secretion of basal insulin

But the control depends upon the available intact pancreatic betacells.

Secondly metformin alone when given will not be sufficient to control blood sugars effectively.

Thirdly you have reached more than half the dosages that is recommended per day. So raising glimeperide will be of no use.

So it is adviced to add basal insulin
