First of all I would like to know about the weight and height and food habit of your son.At 5 years average weight around 18 kgs.The height of any one is depends on parents average height.If your sons weight is less than 80% of average than its called malnutrition.
Constant and repeated cough is commonly may be asthma and should be treated properly and its may be a cause of malnutrition like other chronic disease.

As he is having constant cough and not gaining weight,he requires some investigations and after reports go for treatment accordingly.
At this age you should rule out,
1,Worm infestation,
2,Anemia due to worms,
3,Primary cvomplex as he is having constant cough and not gaining weight,
Go for ,
1,CBC,Hb,ESR,
2,Montoque test,
3,Stool test for ova avnd cust.
Give high nourishing food,Iron and multivitamin supplements,
More milk,pulses,eggs,cheese,dry fruits.
