Hi, Dr. Grief: My name is Leidiana Herandez and I m a pharmacy intern. We have a patient that has been on Cipralex 20 mg for 7 weeks and we need to switch her to Remeron RD 15 mg due to treatment failure. Could you please tell me in detail how should I taper down Cipralex? Thanks

Hi, sorry i am not Dr Grief, this is Dr Vaddadi answerring you.
Tapering ciplarex should be done over 4 weeks duration.
First, reduce the dose of ciplarex to 15 mg for 1st week.
At the begining of second week, make it 10 mg and start the patient simultaneously on 7.5 mg of remeron.
3 rd week - Reduce ciplarex to 5 mg and increase remeron to 15 mg
4 th week- stop cipralex totally and continue remeron.
By this method, we can avoid withdrawal symptoms, and reduce the sedation effect of mirtazapine (remeron).
