My Husband is diagnosed with early stages of multiple myeloma and has stage 3 kidney disease. He has protein coming from his kidneys with a reading of 129. He is scheduled to have a kidney biopsy tomorrow. I guess what I am asking do you think he should.
Hi, Yes, he will require renal biopsy provided there is recent worsening of kidney functions or for confirmation of diagnosis of multiple myeloma, as sometimes not all blood parameters are conclusive for diagnosis. Kidney biopsy is a safe procedure, and it gives definate insight into the kidney problem, and it helps the doctor to manage the disease process better.
Renal disease in myeloma most often presents as renal insufficiency and proteinuria. Occasionally, patients with myeloma present with renal tubular dysfunction, including defects in acidification and concentration and, rarely, the Fanconi syndrome. Although proteinuria is observed in >80% of cases, it most often consists of light chains, and light-chain proteinuria can be massive (>10 g/d). Fewer than 15 to 25% of patients with myeloma actually develop the nephrotic syndrome.
A biopsy-based diagnosis is important in the evaluation of patients with myeloma because each of the renal lesions has its own therapeutic and prognostic implications.
