Experiencing Tightness In Muscles When Lifting Load. Having Vitamin D Deficiency. Suggest The Treatment?

Question: Hi,

I sometimes feel my muscles get tight(Contracted) and muscles take few seconds to relax. It happens when i indulge in any physical activity which puts load on my muscle. It happens only for the first time and after repetitive movements muscles behave normally.

For example in case when i start climbing up the stairs. After climbing first or second stair my thigh muscle become tight and muscle takes few seconds to relax. therefore i have to wait for few seconds. When muscles gets normal(relaxed) i can climb the stairs quickly. For rest of the stairs i do not get any problem.

Another case is after sitting on a sofa for a long time. When i try to get up from sofa my muscles got tight and i can walk normally only after waiting for couple of seconds.

I referred below link and visited a neurologist:

I discussed with him answer suggested by Dr. P. XXXXXXX in the link. He wrote few tests can be done for some deficiency although these symptoms might be of myotonia congenita. I have attached the test reports. The Vitamin D test said that i had a vitamin D deficiency. So the neurologist suggested me to have few Vitamin D XXXXXXX nerve functioning tablets ecozyme for two months.

I took these medicines for 2 month but i did not feel any difference so i discontinued .
After that i never visited the doctor. It was January 2012.

I have gone through the reports attached.
Your symptoms are suggestive of myotonia congenita.
Electromyography(EMG) test and muscle biopsy is required to reach the diagnosis.
In EMG electrical activity of muscle is checked.I suggest you to consult the neurologist again. After confirming your condition your doctor might start some medications to treat your symptoms.

Few things i would like to mention:

1. Few years back(2008) i noticed one thing that for almost 1 or 2 months i had not felt any problem. For that period everything was normal. I was following my normal diet.

But one thing i remember is during that period i was suffering from throat infection frequently and consulted a few doctors and they referred different anti bio-tics. most noticeably Moxikind CV.

2. I think i have this problem since childhood. I can see similar symptom in my brother. But Non in my father family(like his brothers/sisters/cousins) have similar problem nor in my mother family.

3. The intensity of problem gets increased if i do not have food for long time.
For example after lunch and before dinner(between 6-8). That is why i was wondering if this is some kind of deficiency.

Thanks for the prompt and detailed answer. I further want to clarify a few things. Kindly consider the details i have mentioned in trailing follow up query:

1. If test(EMG) for myotonia congenita is negative. Then i must go for muscle biopsy? Does these tests are normal or will they hurt me anyway? What if both the test are negative how should i proceed in that case?

2. If it is myotonia congenita then is it curable?

3. As mentioned in the link Dr XXXXXXX suggested neurobion tab.
Can you please explain and provide me few tablet names?

4. In another answer Dr. XXXXXXX XXXXXXX XXXXXXX suggested "Get yourself tested for Hypothyroidism,Serum calcium,Serum VIT D,Serum B12,Serum FOLIC Acid ,Hemogram and urine microscopy.Any deficiency subsequently found can be treated easily". It would be great if u can provide some details about this as well.
5. In the same link Dr. XXXXXXX Tayal suggested that it can be "electrolyte imbalance in your body. Taking some antioxidants or alanine supplements will help you"

As i am not from healthcare it would be really help full if you can provide me some details on above mentioned questions.

and sorry for the long list of questions since i was suffering from these symptoms for so long i want to know all possible scenarios before proceeding ahead.

Hi,Sorry for late reply.

If test for EMG is negative, muscle biopsy is advisable. These tests are normal and does not hurt you.
There is currently no cure for the disorder. It is important to learn what triggers attacks of myotonic reaction/muscle stiffness. In your case fasting triggers the attacks.
Painful muscle stiffness was provoked by fasting and oral pottassium intake and was relieved by carbohydrate contaning foods.
It is possible to prepare for more sternous activity by carefully warming up the muscles.
