Question: Hi Doctor,

I am XXXXXX from Bangalore.I had my Cesarean section on Nov 20th 2012.
I was breast feeding till 4th month that is till March 20th later as my milk supply was very low I couldn't breastfeed my baby.

I had post partum bleeding till December 26th 2012 i.e nearly 40 days.
Later I got my first period on XXXXXXX 12th 2013.I thought it was still postpartum bleeding and visited my gynecologist who performed C-section delivery.She said this could be my period and it usually stops within 7 days.
It stopped within in 7 days.

I didn't get my next period for the next month i.e for February 2012.I was still BF then.Then I got my next period on March 8th 2013 and I had my periods for 7 days.
Later after 10-15days I was getting little white discharge with mild itching and irritation.I consulted doctor she made physical examination and prescribed me candid-cl gel and told me there could be mild fungal infection.I used it and it stopped.

I later got my period within 32 days i.e I got my 2nd period on April 8th,again for 7 days of duration.After this period again I had itching for 2-3days,this time again I went to gynae and had physical checkup.she prescribed a suppository called clingen-3 for 3 nights and surfaz-sn cream,however withsurfaz cream my problem was solved so I decided not to use clingen-3.Now again I got my period on May 9th exactly 32 days gap.Today is my 5th day and I am still spotting (morning once and now once)I am hoping my bleeding stops by today.

I always used to get my period every 25 days for 5 days duration before pregnancy and now after delivery my pattern changed since two cycles came every 32 days with 7 days duration.

Can you please clarify if this change in the pattern for 32 days and 7 days is normal?
or does it take some time to settle back to normal 25-28 day cycle after few months?
When should I consider my cycle as abnormal and visit doctor as I am not breast feeding now?

Can I use clingen-3 after my period stops in case I have mild itching?

Also my incision site towards right side is a bit tender and slight pain before periods and some times when I don't have periods.I complained the same to the doctor she said the doctor who performed the surgery might have used a different type of sutures(sorry I don't remember the term she used) which might take 1 year to heal.Is this normal or do I need to go for another examination or tests.

Thanks for writing in.
It is very common for the menstrual pattern to be slightly altered after Cesarean section.
By definition, a cycle length of 32 days is within normal, and bleeding duration of 7 days is also normal.
So please do not worry.
The cycles are normal at present, but it might settle down to your pre pregnancy shedule within a few months.
After stopping breastfeeding, it takes upto 3 - 6 months to establish a proper menstrual pattern.
You should visit your doctor in case of abnormally heavy bleeding, or absence of period for two months or extremely painful bleeding.
Also, having stopped breastfeeding, please use a reliable method of birth control, as you are now at risk of pregnancy.
Clingen is a vaginal preparation for pelvic infection.
It is not recommended to use it without a check up, as the type of infection ( fungal, bacterial, trichomonas or others ) needs to be confirmed before taking medication.
So if you have access, please consult a Gynecologist for vaginal infection.
Uterine sutures are mostly Vicryl these days, and these take 3 months to completely absorb.
The pain, inflammation and post surgical edema would come to normal maximum within 6 months.
I would ask you to go for a complete pelvic check up , and a pelvic transvaginal ultrasound scan.
Also post delivery, go for the following tests ;
1. hemoglobin and complete blood count to rule out anemia.
2. glucose tolerance tests to rule out diabetes.
3. hypothyroidism , as it can lead to abnormal cycles.
4. calcium and vitamin D levels
Stress, anxiety, rapid weight changes, hectic travelling, can also lead to slightly irregular cycles.
For general pointers -
1. Try and gradually achieve a healthy BMI
2. Take regular iron and calcium supplements for 6 months post delivery
3. Get the blood tests done.
4. Maintain a menstrual diary and report to your doctor in case of any abnormality
5. Use reliable birth control measures.
