Can I get you for the consult about grandchild who was put on meds for ADD with only once source reporting: his mother. No evals were presented by teachers. ADD has to observed in two or more settings. He has never had a problem in school with attending behavior or social behavior. The mother has been ill for months with debilitating pain. My son recently received a promotion and has been working overtime with family and job

Although the behavioral changes need to be present at more than one setting, a parent if adequately concerned can present information about how the child behaves in both the settings of home and school. So, a single source reporting by the mother is generally enough.
There is a validated tool called Strenghts and Difficulties Questionnaire (SDQ) which will help you to find out whether the child could be having ADD. You can ask the mother to answer it online at:
