Brief Answer:
can try silodosin

Detailed Answer:
Hi,
Veltam is a good medicine, and it's ingredient tamsulosin has been available for over 10 years now. A drug that has come more recently is silodosin, which is usually regarded as the best of alpha-blockers for BPH, especially in elderly persons as it can cause ejaculation disturbances which might not acceptable for younger men.
It is available as several brands such as silofast, silodal, etc and the usual dose is 8mg once at bedtime after dinner and you can stop veltam and try this for a few days. If this seems better for your father, he can continue this long term.
If your father has a very large prostate, he can combine this with dutasteride, which is slightly more efficacious than finasteride. Again, silodosin is available in combination with dutasteride as silofast -d or sildura.
The back pain and shaking are not likely to be increased due to these drugs.
If he wants to continue with veltam, veltam plus is probably a better option than the older veltam -f.
Hope he finds improvement. This can be measured by a simple test called uroflowmetry which can be performed one month after you start the medicines and can compare with a previous study if available.
Hope I have been of help.
