Brief Answer:
Obesity refractory to dieting & exercise programme

Detailed Answer:
Hi,

Thank you for your query. I can understand your concerns.
You haven't mentioned your height .Hence BMI cannot be calculated.

Slow metabolism may be due to hypothyroidism which can cause obesity.
I suggest to test for Serum free T3,free T4 & TSH level.

Liraglutide, a glucagon-like peptide 1 receptor agonist currently
approved for the treatment of type 2 diabetes, has independent
weight loss effects via hypothalamic neural activation causing appetite
suppression. In adults with BMIs of 30–40 kg/m2 once daily SC liraglutide (1.2 mg) is worth trying. The side effects are minimal. You need to consult an Endocrinologist for that.
Otherwise Bariatric surgery can be considered for patients with
severe obesity (BMI, ≥40 kg/m2).
