The doctor said I might have sleep apnea.
But the test woulld only show damage, I had a suspision that I had pulmnary hypertention,
I was in bed 14 hours a day
so I started taking viagra I got from my friend as recommended and the systmos went away mostly but not completely, would there be any other explanation for an improvement ither than pulmonary hypertention

1. You seem to have exertional breathlessness about a month a ago.
2. Your pulmonary function test was abnormal along with good reversibility. This finding requires to have inhalers.
3. Pulmonary hypertension can br picked up by chest xray as well as echocardiography. If the echo was normal, there are less chances of pulmonary hypertension. A stress test to rule out any cardiac abnormality would help.
3. Yes, Obstructive sleep apnea needs to be ruled out by a sleep test.
4. Taking self medication of viagra for medical purpose is strictly not recommended. Cilias (Tadalafil) is a medicine of the same class as viagra.But in absence of confirmation of diagnosis of pulmonary hypertension, it is strictly advised not to take this medication without prescription from your doctor.
5. You need to follow up with pulmonologist and cardiologist for further workup of your breathlessness.

The pulmonologist said it might be sleep apnea or early asma but that would not explain why the symptoms disappeared after retavio (viagra)
Possibility of early asthma is there.

1. Considering your abnormal pft, possibility of early asthma is there.
2. In absence of an established clinical diagnosis of pulmonary hypertension, it is difficult to say whether pulmonary hypertension responded to viagra since such a dramatic response is usually not seen in pulmonary hypertension. Also primary pulmonary hypertension is a very rare entity.Usually it is secondary to an identifiable cause which is absent in your case.
3. I suggest, you continue the evaluation as advised by your doctor rather than taking empirical viagra. A sleep study may help.

in its early stages the only symptoms are shortness of redth during excercise and cold feet
those who get it diagnosed later are in severe conditions.
furthermore one cause of it is suppression of BMPR2 or Alk1 which could of resulted from a biologic like stallera and would not show up on any of the scans
2D echo can measure pulmonary artery pressures.

Thanks for the follow up query. After going through the query, I would like to comment the following:
1. 2D echo does give pulmonary artery pressure estimates and it can detect early pulmonary artery hypertension or exercise associated pulmonary artery hypertension. I am not aware if your report gives the pulmonary artery pressure estimates. If that has not been mentioned then you can again go for it. The gold standard is pulmonary catheterisation which is an invasive procedure.
2. I would again like to inform herewith that such a dramatic response is not expected in pulmonary artery hypertension. I am not sure about how much symptomatic improvement has been there on viagra but you do not have any evidence of pulmonary hypertension.
3. Stellara is a relatively newer molecule with tiredness and shortness of breath as documented side effects. There has been no evidence of it causing pulmonary hypertension however rare chances of pulmonary embolisms do exist.
4. I would not recommend starting viagra for shortness of breath and would prefer complete investigations including repeat 2 d echo/ ct scan thorax - contrast and plain/ sleep study/ autoimmune panel / thyroid etc. to clinch the cause and then treat accordingly.

1. Stellara is an immunomodulator drug. Hence it predisposes to acquired immunodeficiency leading to opportunistic infections. Also there is predisposition to inflammation and swelling of various structures in respiratory system, pulmonary microembolism,early Pulmonary edema, bronchitis, etc.
2. Any of the above factors can lead to breathlessness apart from other known causes like asthma.
