General Surgeon 's  Response
Hi,

As you have correctly mentioned the stress can cause or enhance lots of problems. The commonest cause of unexplained recurrent fever is hidden infection, the commonest sites being sinuses, urinary tract, liver or lungs. Hence I would advise him the following:
Get the blood tests done, routine as well as culture and sensitivity.
Urine routine, microscopy, culture and sensitivity.
Ultrasound of abdomen.
X-ray Chest.
CT scan of sinuses.

All these along with clinical evaluation by your doctor can pinpoint the cause and get treated specifically. As for sleep apnea he has to consult an ENT Surgeon trained in OSA for evaluation, investigations and get treated as advised, may need surgery. Weight reduction and avoiding stress helps further.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
