I read your question and i understand your concern. With history of jakafi use , indicates you have some myelofibrosis illness like polycythemia causing splenomegaly and that putting pressure effect on stomach and thus causing gastric discomfort. At age of 81 i am afraid there is not much one can do. Supportive care, gastric pain medicines, some antacids, nexium, small frequent meals etc are commonly helpful.
Splenectomy can still be considered using minimal invasive surgery provided the anesthesia fitness is evaluated. Talk to your oncologist / hematologist and let them decide what is best for you. Stomach pain just needs supportive care.
