What Causes Central Vision Diplopia?

Question: I have developed central vision diplopia
from middle field down
aggrivated by lateral head tilt to the right head rotation to the right and head extension
no blurring of vision
long sight better than 20/20
ct mri says no cause for damage
Neurologist say not a neurological cause
any ideas?
eyes clear fundas etc
prior histort la forte 2 fracture 2006 left side of face hardware placed under left eye
2012 had hit in face ,had the plate removed under left eye due to the plate being twisted. no new plates were reinserted
Had a fall down two steps in Oct 2015 caused 2 disc injuries L4 and L5 both Post central slightly to left.
When I fell down the steps ,I had to look down to see the steps my vision was little slightly blurred (only in down position)at foot distance (176cm )caused over step then fell to the ground.
THEN 3.5 MONTHS later developed central vision diplopia (diplopia when two eyes open ) no diplopia with one eye open.
Drugs .Been taking /APO escitalopram for depression from before fall in Oct up untill developing diplopia. Stopped now
Tried Lyrica for leg pain (POST FALL LAST YEAR) ,but did not like it, for leg pain.
Pre double vision I started to sneeze a little ,made a appointment to see ENT Dr. due to possible sinus infection Headache and the sneezing. Had 3 turbinectomies prior ,but post 2006 Laforte 2 fracture
Also had a MVA December 2014 ,not major however whiplash,also little one Aug 2015 rear ended the result lots of headached ,but moderated since XXXXXXX
At this time I get a pain above right eye PIN POINT and lateral around temporal area of head. Been taking Antiobiotics,(I think possibly due to eyestrain due to diplopia,but was having head pain pre double vision)

My dear,
Thanks for sending your query to us.
After going through your history in detail my impression is that you have developd vertical muscle paresis (wekness) or paralysis. This can be confirmed by orthoptic examination and Hess's screen charting, which again is done by the orthoptist.This checkup is to be ordered by your eye doctor.
Prism glasses are only used when we don't expect/get improvement. By giving prisms,as they are like crutches ,natural improvement is not possible.
You have to get examined by eye doctor to rule out muscle paralysis and manager accordingly.

My dear,
Thanks for getting back to me.
This problem, which my clinical impression suggests to be due to muscle paresis/paralysis has to have a reason. That cause has to be investigated.Ophthalmologist, neurologist and an internist have to decide jointly about investigation and results.
My one objection is use of prisms,they do not allow the muscles to regain power as they supplement muscle strength.
Treatment of disease depends upon the cause.So we have to focus on the cause and treatment.

3 4 and 6 cranial nerves involved
If one of these nerves involved would that not restrict eye movement as well in the different directions of gaze

3,4,6 nerve involvement in the brain is a lot of area and there should be a reason for this.That reason is to be investigated and cured.
That nerve involvement has recovered so your eyes are moving all around but some weakness remains so there is diplopia.
look into the cause of this 3,4,6 nerve paralysis.
