Question: HELLO SIR!i am XXXX,2 days ago my brother got seizure,after mri done today,he is diagnosed to have inflammatory granuloma...neurocysticercosis,currently he hs been prescribed with omnacortil,carbamazapine,and lobazam.cud u plz throw some light on the prognosis and the prognosis of treatmnt and the chances of having a seizure again....thank you

I have read through your query in detail.
Your brother has a condition due to the presence of tapeworm cysts in the brain. This is a common condition and is causing seizure in him. The tapeworm dies and forms a granuloma which is described as an inflammatory granuloma. When the granuloma gets slightly displaced in the brain at that time there is abnormal electrical activity and mild swelling in the brain tissue. This gives rise to seizures. The duration and frequency of siezures depends on the number, location and the electrical threshold of the brain. Taking medications will increase the brain threshold and this will reduce the seizures.

However, sometimes seizures can occur if he stops taking the medications. This is because the brain threshold will once again be lowered and small current bursts will be generated which will take the symptom of seizures.

He must continue taking medications for at least 2 years and the prognosis is good. He must avoid driving, swimming or working with heavy machinery for 6 months. This is because if he gets seizures when doing a high risk activity then there is risk of injury to himself and those around him.

Please see that he takes medications every day and washes hands and eats hygienically cooked food.

thank you sir for your expert opinion.i still have a doubt about the chances of getting a seizure again what could be done to minimize it to zero?and ofcourse the side effect of the drugs and to minimize them.

Thanks for writing back with an update.

The medication omnacortil contains prednisolone which is a steroid and is given to reduce any brain swelling. This must be taken only for few days as prescribed by the doctor. Taking if for long time can have side effects.

Taking carbamazepine and lobazam are to reduce seizures. they might have to be taken for a longer period of time and this has to be done to control seizures.

Taking medications carbamazepine and lobazam will increase the brain electrical threshold and prevent seizures. Please follow up with the doctor as required, at monthly intervals in beginning and 3 to 6 months after that. It is important to be under doctors guidance for at least 2 to 5 years. Please do not worry.

my last query would be...can he take deworming drugs every 6 monthly now onwards to prevent any parasite infections? adding to the reports.....it was in the temporoparietal area.

Deworming medications can be taken every 6 months. More important is to eat properly washed vegetables and cook meat in pressure cooker. Avoid pork meat if he takes it.

I have seen more than 500 patients with such problems and there is not much to worry if there is only a single one.

He should continue taking medication as per doctor and if there are seizures then keep record of events and inform doctor in next visit.
