Hello,
as you probably already know the presence of Leiden factor increases your risk for thrombosis (blood clot formation). Most patients with Leiden factor (particularly if you're heterozygote, that is if you have only one defective gene) won't suffer any thrombosis during their life and do not require treatment. Since you're taking xarelto, I suppose that you've already suffered thrombosis or are a homozygote (both genes are defective).
Traveling, particularly if it requires prolonged immobility - like sitting on the plane - may increase your chances for a blood clot and your nose seems to have a bleeding tendency that the ENT may manage. So taking everything into account I would suggest a new visit to the ENT for a more permanent solution. Stopping xarelto may help transiently but it increases your risk for thrombosis and won't help in the long run.
