Question: Girlfriend complains of a tearing like pain during intercourse. Was examined with no tears and minor cervix redness. No sores or other abnormalities. What could it be

There are so many causes of pain during sex.
As you said there was no tear present during examination and only mild cervical redness is present the chances of infection can be there in her case.
Cervical redness can be due to infection ( yeast or bacterial infection) or due to inflammation.
Can you please give us more details about her pain.
For how long it is there?
Any other associated symptoms like fever etc.?
It starts during deep penetration or it is always be there?
Any type of vaginal discharge?
Is she taking any type of medication?
Sex was protected or not?

Wouldn't a yeast infection have been more obvious with the examination of the internal part of the vagina? She has once bled. The pain has been happening only during missionary and pretty deep thrusts. It seems to be fine with her on top or other positions. She has no fever. It was unprotected

She takes birth control but that is about it

It doesn't hurt start on entry but as we get going it hurts. As I said a tearing like pain

The pain is constant through sex. little soreness after

Sorry for the delay in my response.
Yes, yeast infection can be detected by internal examination but only be confirmed by KOH staining.
In my opinion her symptoms are suggestive of cervitis( inflammation of cervix).
You should consult a Gynecologist and get her swab culture test done to rule out pathologic organism.
And she should take antibiotics and pain killer to relieve her symptoms.
