default
Posted on Fri, 20 May 2016
Question: I have had my thyroid medicine changed 3 times in the last 9 months. I was on .1 synthroid for several years since first diagnosed. Now they have dropped it down to .5. I was wondering what could be going on my thyroxine it is high. Could my thyroid start working better or should I see a endocrinologist. I am seeing a PA. I am still having symptoms under active thyroid.

Brief Answer:
Thyroid

Detailed Answer:
Sorry to note your challenges with regulating your thyroid medication.

Thyroid matters can be complex to deal with and it is best to see an endocrinologist in-person for accurate diagnosis and expert management which typically includes proper dose adjustments.

PAs with good endocrine training can do a fair job too. However additional input here from the endocrinologist seems to be in order to correctly diagnose and stabilize your thyroid condition.

When I see someone like you in my practice, I typically order the following blood tests in addition to a detailed physical examination:

CBC (Complete Blood Count, also known as Hemogram; includes Hemoglobin, WBC and Platelet counts)
Electrolytes (Sodium and Potassium in particular)
HbA1c (Glycosylated Hemoglobin = your 3 month glucose average)
Liver function tests (SGOT , SGPT, Albumin, Bilirubin, Alkaline Phosphatase)
Kidney function tests (BUN, Creatinine)
TSH
Free T4
Anti Thyroid Peroxidase (TPO) antibodies
Anti Thyroglobulin antibodies
25 hydroxy Vitamin D
